INFORMATION FOR NEW CLIENTS

What to Expect When You Contact Me

If you are interested in working with me, please fill out the secure contact form below. After receiving your initial contact form, I will schedule a brief 15-minute phone conversation to better understand your needs. At that time, I will either schedule an initial appointment or offer referrals to therapists who might better meet your needs. 

 

Before our first appointment, you will receive an email that will lead you to your account  on my therapy portal where there will be consent forms to read and sign, some brief initial intake documents, a form to provide insurance information, and a form to enter credit card information.  I require a credit card be on file for fees, deductibles, and copays.  Forms must be completed before our first meeting. 

 

All telehealth sessions are conducted via secure, HIPAA compliant Zoom.

INSURANCE AND FEES

Fees:  $200 per 60 minute session; $250 for initial evaluation.  Limited sliding scale available.

 

I am in network with Blue Cross and Blue Shield Traditional and PPO (not Blue Care Network).  

 

If you are out of network, and wish to use your insurance, I will either provide a receipt called a “superbill” which you can use to obtain reimbursement, or in limited circumstances, bill your insurance directly.

 

If you are not using your insurance or do not have insurance, you have a right, under the No Surprises Act  to receive a Good Faith Estimate of the costs of your treatment.
